Chronic Heart Failure Survey

This is a voluntary online anonymous survey to find out more about people living with the condition of chronic heart failure (CHF).  The condition is also sometimes called congestive cardiac failure (CCF).

It is being carried out by Dr Steve Leslie and Dr Martin Denvir, two consultant cardiologists with a special interest in CCF working in Scotland, United Kingdom.

We would like to know more about the experiences of people from different countries who have been given a diagnosis of congestive heart failure. We hope this information will ultimately help doctors to improve patient care.  If you are suffering from Chronic Heart Failure then we would very much like you to complete the survey which consists of 12 questions and should take only a few minutes to complete.

No identifiable data will be asked and participation in this survey is completely confidential and secure. However, you may give us your email address if you wish to find out the results of the survey but this is entirely voluntary.
